Scenario:
You are still the Director of a Health Department in Somewhere County, population 19,000. Your county has the highest incidence rate of colon cancer in the State of Kentucky as well as the highest mortality rate because of late diagnosis. Your county is also among the top 5 medically underserved counties in the State. The nearest hospital is 90 miles away in another county.
The majority of your countys population consists of poor farmers who raise tobacco; about 15% are Amish farmers. Geographically, the town and its health department are in the far northeast section of the county. Most farmers would have to spend at least 1 to 1 ½ hour to drive in to town.
The State Health Department Director remains very concerned about the unusually high mortality rates. Using state money received from a prevention and early detection grant from the Centers for Disease Control, Somewhere County will $150,000 to conduct colon cancer screenings and other prevention and education activities. This money is to be spent by the end of 2018.
As the Health Dept Director, you will be able to borrow a mobile health van from the WKU Rural Health Institute free on Fridays and Saturdays but would have to pay for a WKU certified driver and $.45 mileage rate for round trips each time the van is used. The mileage between WKU and the health department is 140 miles one way and does NOT include mileage for going to small communities around the county.
You also found that occult blood kits (a foldable cardboard cad on which a smear of stool is tested for blood) cost about $2.50 each. His department has 1 health educator, a nurse practitioner and a part time nurse as well as a part time member with background in nutrition. The full time staff are state employees and not permitted to work on Saturdays or for extra pay. There is also one family practitioner in town who has 2 nurses who work part time and might be willing to work for extra pay.
